Can just the valve be revised or will a new pouch/valve have to be created? I would prefer not to lose any additional small bowel, if someone could just reconstuct the valve.

Yes, I believe so. Bu, it may depend on what is actually wrong. Do you have a surgeon - not sure where you live. I had my kpouch in 1979 and two revisions but all fine after 1983 - they did not need to redo the entire pouch.
