Continent urinary incontinence

Hi everyone. I need help. My son is 27 and when he was 4 yrs old, his dad accidentally ran over him causing a pelvic fracture that resulted in trauma to his urethra. Now @ 27 after several attempts to restore natural function, he has the continent urinary diversion, which he has had for close to 15 years. The problem is he is having a lot of leakage from his stoma. He can catheterize his bladder pouch and 15 minutes later he leaks all over himself. It has become a huge problem and one that we are not sure where to turn to for help. His urologist is not very helpful anymore. He also has awful pain in his urinary system that is driving him crazy. Has anyone had to have a surgical repair of the incontinence issue?? Thank you in advance for your help.
