Kock Pouch - Help for my mom
I'm seeking help for my mom who's 84. She's had her Kock Pouch since 1984. The initial surgery was done at USC Kenneth Norris Hospital in Los Angeles by Dr. Donald Skinner. Since then she's had several surgeries to remove stones but that's been the extent of any issues. Now however, she has become very incontinent. After 2 to 3 hours urine will begin spilling out of the stoma. No one here in Albuquerque New Mexico knows what to do with a Kock Pouch. The doctors here told her that her only alternative is wearing a foley the rest of her life. Well, she tried the foley and that's no longer an option. I was recently advised to seek assistance from a training university hospital and to request the head of urology. Well guess what, that's who we've been seeing all these years! She's going to be 85 and is too old and unhealthy to have surgery. Is there something that can block the urine from coming out...like some kind of plug? Can someone please guide us? 2022 Update of my post: I am DEFINITELY NO LONGER RECOMMENDING Dr Pokala Kiran in NYC and suggest keeping away from him.

The term "Kock Pouch" is ambiguous, as it can refer to either a fecal or urinary diversion (which share the same name because both were developed by Dr Nils Kock of Sweden). Dr Kiran may be an excellent surgeon for the fecal Kock pouch, but may not be able to help with a urinary Kock pouch, which is presumably what Laurie's Mom has.
In looking for doctors who can help with an incontinent urinary Kock pouch, it may be helpful to also look for doctors familiar with the "Indiana Pouch" which is another variant of continent urinary diversion.
Meanwhile, Laurie, given that your Mom is 85 and isn't a candidate for surgery and the foley isn't an option, I wonder if she could simply wear an ostomy pouch of the sort that would be used with a standard urostomy. Pouching might be difficult, as the stoma may be flush or recessed, but the best person to help with such issues would be a Wound Ostomy Continence Nurse (WOCN) rather than a physician.
