Entering the working world with a colostomy (not sure of the right place to post this)

I had been on disability with Crohn's disease not too long after high school graduation, had a colostomy in 1999, but currently in remission (even though it is still possible for flareups of the disease). I recently had a re-evaluation, and although SSA states that I would have some limitations, I should be able to work, so my benefits will stop completely. So that is what I am setting out to do, and seeing that I am in remission, I welcome the change. I am confident that I can eventually find employment, but the issue I am facing is that since my Social Security benefits will stop, so will my health insurance. Therefore, until I can sign up for a health insurance through a place of employment (which usually takes a while to come into effect), I will need help to be able to get my medication (in order to remain in remission) and my ostomy supplies. I could purchase some of this equipment out of pocket, but not enough to last, and the supply count that I would require (plus my medication) would be cost prohibitive seeing I have other expenses.
Is there anyone who has some ideas on how I can work around this so I can get my much needed ostomy supplies and medications?
Also, if this is not the right place for this post, I apologize. I just feel it does because it concerns (in part) my ostomy supplies which I cannot be without.
