"Pull Through Method"...advice for a newbie

I am new to the forum, my name is Doug and I will be 21 in March. I have been dealing with ulcerative colitis for almost 5 years. I am at the point in my disease where I am considering having a ileoanal anastomosis "pull through" surgery done (if I am eligible for that kind). Is this always a 2 part procedure? Will I have an ostomy of some kind, and if so for how long? I would like to hear some responses from real people who also are dealing or have dealt with the same things as me, and have had the surgery. There is so much information out there to read. I was so glad to have stumbled upon this site, so I can actually talk to someone besides the doctors :!:

a straight pull-through isn't always done as a 2 part surgery. a woman i know had it done with one surgery and it worked fine for her. my experience when i had my colon removed was that i'd have an ileostomy for about three months (over the summer) and it'd be reversed before school started up. I ended up having a straight pull through done although they first tried a jpouch. hope that helps to answer your ?'s and good luck

Hi Doug,
I have had my J-Pouch for 16 years and had the surgery done in two steps. As far as I know the sugery is very rarely done in one step due to the fact that the connection of the small intestine to the rectum needs to heal for about six to eight weeks. So, you would have an external ostomy for about the time mentioned above. I have met only one person in the whole 16 years that I have had my pouch that has had the one step procedure done.
I can suggest that you might want to log on to a website called jpouch.org. as well as keeping us posted here at UOAA on your progress. I have found over the years UOAA and Jpouch.org have been extermly helpful because of the vast knowledge of the people of both sites.

Hey there;
I had a total colectomy in 1995. I had emergency surgery and the doctor was not sure if he would/could set me up for the j-pouch. I was very lucky to have such a good doctor, he set me up for the j-pouch procedure and I didn't have any problems with my stoma! This paticular doctor dod not do the j-pouch surgery so he had to refer me to a doctor in another (larger) city. O.H.S.U. to be exact. My doctor is/was Karen Deveney and she has been doing this type of surgery for 7 years before she did mine. I was taking steroids at thr time of the colectomy so I had to have a three part surgery. 1st the colectomy, 2nd the j-pouch build up, 3rd the hook up. The first surgery was performed on March 17th. The second surgery was performed on September 25th. The third surgery was performed on December 10th, all in the same year! Yes it does seem like a long time to go through the whole process but let me tell you one thing, you should not rush this because if you do get one step of this procedure done before you really should it could be a very bad thing. There is so much that can go wrong, just read some of the letters people have written on the other part of this site. Believe me you will have no problem waiting until everything is good to go! I have heard that you can have this surgery in 2 stages but I wouldn't reccomend it, especially if you are really sick now or if you have taken steriods for a number of years. The most important thing that you can do for yourself right now is do the most in debth research you have ever done on the doctor that you are planning to have do the surgery. I really mean this, there are some that really don't know what they are doing! I know if you post in the other issue on this site and ask you will get many replies on who had what doctor or who other people would reccomend you go to or not go to. Just don't rush into something unless you are on your death bed and have no choice.

I forgot to mention something and I really want you to know this. If If or when you have this surgery one thing you should know is that the doctor may decide to make the incesion from your sternum to your pubic bone and I want to stress to you that you can request that the doctor not to make the incision so far up! They can perform the surgery just as well if they go from your belly button to your pubic bone, it is just takes a bit longer for them to get access to your (stuff). Just thought that I would throw that in. I wish someone would of told me about this before I went under the knife! Oh well if I can help another out then there is a reason for it.
