Hello from Australia, I had a J pouch(after 1 year of illeostomy bag)created 17 years ago, it has been very good, after all the hassle of ulcerative colitis, but just the last few years it has been too active. The doctors here offered to do a sacral nerve implant that is attached to your spine and it releases the bowel muscles by working a magnet inserted in your body somewhere. The specialist said it did not stop the pain of the J pouch going into spasm or the loose bms. Have tried all the usual meds by the truck load, the best one for me is Panadeine Forte, 500mg paracetamol and 30mg codeine, it slows things down alot. I decided not to have the implant, as I have control for quite a time, but am really sick of it running my life. Looking around the net I saw all this information on the Kock Pouch, this has never been offered to me in Oz, and I can't find anyone here that would do it. I intend to ask my specialist when I see him in a month, so just thanks for the info, I hope someone here has heard of it.

Hi, i'm new to the forum, my name is Heather, I had a J pouch created in Oz by Dr. Borton,17 years ago, and it has been wonderful, especially after 20 years of pain from ulterative colitis. Unfortunately, lately I have had alot of trouble with it, mainly just being too active and also I've had alot of stress, which I think made it worse, and caused more stress. You are all very lucky in the US as you seem to have access to surgeons that will perform the BCIR Kock pouch, I have been told if I don't want to keep the J Pouch I have to go back to the external appliance, and I certainly dont won't do that. I printed some info from the net on
the about the BCIR modified Kock pouch, and hopefully I'll get a positive response. Let you know how it goes.

Just to mention for those who know Dr. Fazio-- he is apparently very sick again. He's a great surgeon, for sure, and I wish him all the best.

As for anyone considering the K-pouch- I would really advise against it. It can be very nice and give you a little more confidence, but I truly don't think its worth all the health risks, emergencies and initial surgery that come with it. I just lost mine after 14 years, and although such time may be considered a long run, the amount of danger it has put me in over the last four years, along with recent failed surgery at the Cleveland Clinic (Kirin, not Fazio) has disrupted my life on every level. The nipple valve herniated a few years back but was incorrectly diagnosed as a muscle tear. Subsequently, the pouch would kink on me, creating a bowel obstruction that completely shut down my GI. To make matters more complicated, I was living overseas at the time. Being tethered to Cleveland or any of the other few places that deal with the K-pouch is expensive, time consuming and exhausting. Also, most of these places are making far fewer K-pouches each year as the J-pouch has apparently become much more in fashion. This means 10-15 years down the road you'll have less and less doctors available that are knowledgeable about it. Anyway, in the end, I just don't think its worth it, because somewhere down the line you're likely to have problems, and may even lose it like I just did. I think being strong and healthy while living with an ileosotomy is a better choice.

The jpouch is the gold standard and is usually performed first. But, a jpouch isn't an option for those of us that lost our rectums. And, for those who have a failed jpouch, a kock pouch or bcir is a wonderful option. Yes, fewer surgeons may perform it now, but that doesn't mean more young surgeons will not seek to learn these ostomies in the future.

There are many jpouch successes, but many who suffer butt burn, accidents and other problems. A kock pouch does require use of a catheter inserted into a stoma. The end ileo for some like me caused severe allergic reaction causing a bag change daily and it did not adhere to my skin. It's a trade off.

I think an end ileo works for many and ultimately the end result if no other ostomy is doable. I find it sad that in this day and age, we still have limited colorectal surgical options.

I am celebrating my 30th year with my kock pouch in Feb. '10.

Dana Launer in San Diego is fabulous. He did my kock pouch. there is a Dr. Terdiman in San Francisco too. I have not seen him but he is the ONLY one in the Bay area that deals with Kock pouches.
Has anyone had problems with Vitamin D absorption?

Sad sad news. Dr. Dana Launer has passed away. I just saw the obituary a few days ago. He does a partner that he has been training for quite some time - Dr. Woolsey (or something close to that).

Yes, I am very sorry to say Dr Launer passed away. He contacted me a few mos ago with the sad news of his illness. Yes, Dr Worsey has been thoroughly trained and has done many kock pouches.
